Echo Shard gives you a 12% chance to spawn an extra XP shard whenever you kill an enemy. No hidden mechanics.
The extra XP shard appears as purple in game (blue/teal is the normal XP shard)

How it Stacks
Each copy of Echo Shard gives +12% chance.
The chance scales linearly based on how many copies you have:

Example:
Above 100% gains you nothing extra. Although sometimes you will get 2 purple shards from enemies instead of 1 blue and 1 purple. This has no impact other than visuals though.
